Foundation 96's mission is to provide a safe and supportive care community, consisting of multidisciplinary service providers, for people and their caregivers who are transitioning from active cancer treatment to everyday life.
If you are dealing with lingering side effects, take Foundation 96's free Personal Care Plan assessment. It will take you less than 5 minutes to complete the assessment, and you'll be emailed the results of the assessment with suggestions about the types of professionals that can help you manage or eliminate your side effects.
To dive deeper, Foundation 96's health consulting services will allow you to talk with one of their staff about issues you are currently facing and to learn the specific types of professionals that you should seek out for treatment. You can use that information to find providers on your own, or you can connect with Foundation 96's worldwide network of health and wellness professionals.
